Clinical Pharmacist

Job Description:

This position demonstrates consistent application of knowledge and skills to deliver appropriate, safe, efficient and cost-effective pharmaceutical care within the scope of their assigned practice. This position is responsible for the care of patients as well as the development of effective, professional relationships within the multidisciplinary team. The Clinical Pharmacist - Informatics role supports pharmacists, physicians, nurses, and other clinicians as the resource for requests related to EMR or other electronic system changes and additions. This role demonstrates independent decision making as appropriate in evaluation of requests for system improvements and determines feasibility and advisability of system changes. This role works closely with clinicians, clinical programs leaders, clinical services leaders and staff to clarify needs, prioritize requests, and design workflow to fit with electronic systems, and execute changes and facilitate adaptations to the EMR.

Essential Functions

Reviews patients' medication order for allergy interactions, duplication of therapy, appropriateness of drug therapy, and drug- drug interaction. Makes appropriate evidence-based interventions to optimize medication use. Ensures safe, appropriate, cost-effective therapies and accurately dispenses medications.Oversees and/or participates in assuring appropriate medications are prepared and delivered timely to care areas and are stored both securely and appropriate for both stability and effective workflow.Provides counseling to patients and promotes quality medication education and adherence. Ensures patient is well informed, treated with respect, and receives the highest level of professional service.Completes all regulatory and compliance training and ensures documentation of regulatory requirements is clear and complete. Is knowledgeable and compliant with state and federal laws, regulations, pharmacy policies, procedures, and standard operating procedures.Supervises and directs activities of interns, technicians, and support personnel as required. Provides medication related clinical consultation and clarification to other health professionals.Takes the initiative to solve problems including, but not limited to, equipment, maintenance, drug availability, staffing issues, and extraordinary patient care concerns.Identifies potential areas for improvement including processes that could be streamlined or revised to improve patient satisfaction. Identifies potential safety hazards that may create problems for patients and/or staff and takes appropriate actions. Participates in departmental meetings, contributing ideas or feedbackSupports clinic or hospital and system directives, including clinical, compliance, business operations, standardization, and customer service.Supports local and enterprise pharmacy leadership initiatives.Responsible for systems and processes including, but not limited to, computer provider order entry, drug admin pumps, automated dispensing system and compliance with regulatory standards.Coordinates the integration of the pharmacy computer systems with other clinical areas. Trains and supports departmental super-users and end-users. Supervises maintenance of the coding and billing information for patient pharmacy charges.Identifies and coordinates implementation strategies of changes to current software packages. Develops project plan and implementation strategies for new software modules. Directs the process of determining priorities with appropriate users and pharmacy staff.Oversees Pharmacy IS vendor relations, monitoring and following up with vendors on outstanding calls.Develops, reviews and revises policies and procedures for new and existing functionality/operations within pharmacy systems for both interdepartmental and intradepartmental functions, assuring compliance with regulatory agencies. Responsible for ensuring system designs appropriately address patient safety regulations and compliance issues. Collaborates with regional and system counterparts to advance the effectiveness and integrity of pharmacy information systems.

Minimum Qualifications

Doctor of Pharmacy or Bachelor of Science in Pharmacy degree from an accredited institution (education verified).License to practice pharmacy in the state of practice and dispense controlled substancesCurrent knowledge of state and federal regulations and statutes regarding the practice of pharmacy in the state of practice.Effective communication, people management, and project management skills.Immunization certification if applicable to practice site needs

Pharmacy Informatics CertificateInformatics experience, familiarity with Epic, Alaris pumps, and OmnicellPrevious experience working in an integrated healthcare delivery systemBoard CertificationHealth System Residency

The hourly range for this position is listed below. Actual hourly rate dependent upon experience. 

$59.86 - $87.98
