Overview The Director position is responsible for developing and operationalizing the UF Health St Johns strategy for meeting the social health needs of the communities served. This individual is tasked with achieving profitable growth, clinical excellence through community relationships and foundational programs including Care Connect+ and BRAVE. Collaborates with community leaders to assess the health needs of the community and design programs and services to maximize resources to address those needs. Seeks community partnerships to maximize resources and impact on the greatest community health needs. Ensures resources and programs are made available to address community health improvement challenges and needs Responsibilities Responsible for establishing and maintaining high quality patient care standards and services from intake/assessment to program exit at each Care Connect + location, including environment of care, compliance, and regulatory requirements. Identify, apply for and manage grant funding opportunities to support grant management initiatives, ensuring alignment with departmental and organizational goals. Develop compelling grant proposals, oversee compliance with funding requirements, and track financial performance to optimize resource utilization. Collaborate with interdisciplinary teams and external partners to enhance funding prospects and sustain program growth through strategic financial planning. Ensures departmental policies and procedures align with current regulatory requirements, accreditation standards, and best practices. Leads and directs the financial management of the Service Line to ensure desired fiscal status. Develops departmental staffing, revenue and expense budgets and assumes direct responsibility for daily monitoring of labor and non-labor resources including responding to changes in the department business which may affect the ability to achieve the budget goals. Ensure financial health and long-term sustainability of Care Connect + through innovative strategies to grow and maximize the impact made by Care Connect+ Provider Network. Coordinates activities of the department with related activities of other departments to ensure efficiency and economy. Reviews financial data related to each admission and assures initial documentation is completed. Leads business plan development process for new programs and services. Completes monitoring and prepares reports for leadership team related to financial metrics on all aspects of the service line. Ensures the department conforms to all applicable standards and regulations of The Joint Commission and other local, state, and federal agencies having jurisdiction over UF Health St Johns. Oversees all aspects of the UF Health St Johns’ community benefit program to ensure compliance with all IRS requirements. Oversees and directs hiring, training, counseling, evaluating, and management of staff. Creates a positive environment where employees can excel in the delivery of exceptional service and fosters employee participation, satisfaction and organizational commitment. Builds a learning environment that enables employees to realize their highest potential. Ensures all policies, procedures and practices are applied in a standardized manner, maintaining consistency for all staff. Facilitates orientation of new employees regarding components of the department. Facilitates formal and informal learning experiences for staff through identification of staff needs. Conducts department meetings and other communication means to disseminate information, discuss plans, and outline new goals and objectives. Directs all community outreach activities, financial contributions and sponsorships related to Care Connect + and Community Health. Foster and maintain effective relationships with community leaders, Partner Network and funders. Tracks time and attendance and assures compliance with the UF Health St Johns Time and Attendance policy. Qualifications Bachelor's degree in Business, Community Health, Social Work or related field required. Master's preferred. A minimum of 3 years progressive leadership in a related role required.
