Job Description:
We are seeking a dynamic and motivated liaison to join our team. In this role, you will serve as a
key representative for the Clinically Integrated Network (CIN), collaborating with the Medical
Director and Director of Quality and Care Management to develop and maintain strong
relationships with member physician practices.
Day-to-Day Responsibilities:
● Practice Engagement: Visit physician practices regularly, adjusting frequency based
on priority and specialty (Primary Care: regular meetings and pop-in visits; Specialists:
ad hoc meetings).
● Training & Education: Conduct training on our population health management
platform, Lightbeam, and provide education to clinic staff on CIN priorities and quality
initiatives.
● Performance Reviews: Assist in monthly meetings with providers who require a
performance remediation plan, ensuring quality and cost goals are met.
● Resource Management: Maintain a library of resources to share with practices to
support value-based efforts and quality improvement.
● Quality Measure Improvement: Collaborate with clinic nurses and medical staff to
streamline processes that improve quality measure performance.
● Communication: Update physician offices on key changes from payors and CIN to
ensure practices remain informed and aligned with industry standards.
● Collaboration: Work closely with the Director of Quality and Care Management on the
implementation and accomplishment of key quality and cost objectives.
Qualifications:
● Bachelor’s degree required
● Previous experience in healthcare, quality improvement, or population health
management preferred
● Strong interpersonal skills with the ability to work effectively with a wide range of
stakeholders, including physicians, administrators, and healthcare staff
● Ability to work autonomously and manage time effectively while prioritizing task
