RN: Educator Patient

**Job Summary**
The Patient Educator is responsible for developing and providing educational programs and resources to patients and their families, empowering them with the knowledge needed to manage their health conditions and make informed healthcare decisions. This role collaborates with healthcare teams to deliver comprehensive education tailored to individual patient needs.
**Essential Functions**
+ Develops, implements, and evaluates patient education programs and materials based on best practices and patient needs.
+ Provides one-on-one and group education sessions for patients and families, ensuring understanding and retention of critical health information.
+ Collaborates with clinical staff to integrate patient education into the overall care plan and support continuity of care.
+ Assesses patient learning needs, preferences, and barriers to learning, and adapts teaching methods accordingly.
+ Creates educational content that is culturally sensitive, age-appropriate, and aligned with current medical guidelines.
+ Assists patients in understanding complex medical information, such as disease processes, treatment options, medication management, and lifestyle modifications.
+ Maintains documentation of patient education activities and outcomes in the medical record in accordance with hospital policies.
+ Performs other duties as assigned.
+ Complies with all policies and standards.
**Qualifications**
+ Associate Degree preferred
+ 0-1 years of experience in a clinical or healthcare education setting required
+ 1-2 years Experience in developing and delivering patient education programs preferred
**Knowledge, Skills and Abilities**
+ Strong communication and interpersonal skills to effectively educate and engage diverse patient populations.
+ Ability to assess patient learning needs and adapt teaching methods to meet those needs.
+ Knowledge of health literacy principles and the ability to create educational materials that are easy to understand.
+ Ability to work collaboratively with healthcare teams and advocate for patient education as an integral part of care.
+ Familiarity with current medical guidelines and evidence-based practices in patient education.
+ Proficient in using technology to create and deliver educational content.
+ Organizational and time management skills to manage multiple education initiatives and track patient progress.
+ Sensitivity to cultural and social differences that may affect learning and communication.
**Licenses and Certifications**
+ LPN - Licensed Practical Nurse - State Licensure preferred or
+ RN - Registered Nurse - State Licensure and/or Compact State Licensure preferred
+ Certification in Health Education, depending on the area of practice preferred
