Job Description:

Joining the Stabilization and Mobile Response team is an exciting opportunity to be part of innovative client centered care. Stabilization and Mobile Response is a partnership with the Department of Health and Human Services that offers in-home support to children and families experiencing mental health challenges. Our program offers evidence-based models to support families through over the phone triage and coaching, in home risk assessments, and ongoing therapy. Working at Primary Children's Hospital involves access to a multidisciplinary team, ongoing training opportunities, and professional development opportunities.
As a member of our Stabilization and Mobile Response (SMR) team, you will respond to mental health crises both over the phone and in person, including visits to clients' homes and community locations such as schools. You will work in teams of two, consisting of a clinician and a Social Service Worker (SSW). The SSW gathers information from caregivers while the clinician conducts assessments. You will coordinate with emergency services, law enforcement, and other relevant support services, and support case management by identifying and securing resources for patients and their families. During stabilization services, you will collaborate with the clinician to plan and prepare skill-building and engagement activities. Additionally, you will complete intake assessments over the phone with families interested in stabilization services, ensuring comprehensive support and follow-up care.

Job Essentials

Organize and prioritize daily work by reviewing new, current, and discharging patient needs in area(s) of responsibility. Complete documentation as required.Gather psycho-social information for patients and caregivers.Educate, communicate, and coordinate with multiple patient care teams and outside agencies as necessary to ensure the patient receives quality care, a timely intake and discharge, and to identify and resolve delays and issues.Acts as an advocate on behalf of patients and families, where appropriate, to ensure effective communication with multidisciplinary care team, outside agencies and other resources as necessary.May provide initial intake, crisis intervention, and adjustment to illness counseling services as well as appropriate referrals to treatment and/or resources.May identify patients at risk for potential avoidable delays, extended stay, readmission, financial or complex discharge needs. Coordinate with care providers and leaders to reach optimal solutions.May maintain an active role in denial prevention. Proactively intervene with payers to prevent inpatient denials. Communicate any necessary information to payers to help appeal existing denials.May coordinate the discharge planning process from patient admission to departure. Screen for and identify patient needs, develop a discharge plan, and coordinate with internal and external services for care during and after the hospital stay.Provide support and education to patients, families and staff members on Advance Directives.


Minimum Qualifications

Bachelors degree from an accredited institution (degree will be verified).Current state licensure as a Social Services Worker.Computer skills (email, word processing, and spreadsheets).

The hourly range for this position is listed below. Actual hourly rate dependent upon experience. 

$28.15 - $43.42
